I'm a former lifeguard instructor who used to teach those classes, so I can tell you from experience that you shouldn't worry about it. Sure it might have been embarrassing, but I taught classes where some of the trainees couldn't dive 12 feet down and couldn't do spinal injury saves properly without constant instruction. One minor slip? You'll be ok. I hope you recover quickly.
