I go through this allot with explaining to people about my mom and her committing suicide... Never easy, but you learn never to hold it against people who ask
I'm sure, if he was ready to date, he was ready to face questions about his past.
You shouldn't feel bad for that, I think you asked normal questions for a first date and there's no way you could have known all that beforehand. Don't beat yourself up about it. :)
He will get those questions from anyone, don't worry about it, you did nothing wrong, it could happen to anyone, and unfortunately he is going to have to accept that people are going to ask.

No way you could have known, and family usually comes up on one of the first dates so he should expect it and be okay with it. It sucks to feel bad like that though.
He would understand completely. My mother also passed away from breast cancer, and I am not offended when someone mentions my mom. We understand it is not something obvious to others. On a side note, I hope overall the date went well.
At least he didn't get up and leave right?
