I had an orthodontist/dentist who would watch sports from a tv in the corner of the room whilst doing your teeth. Any goals scored would result in him gensturing with his hands, even when in your mouth, tugging.

Call him out on it. Tell him how unsatisfied you are with him and his work, then find somewhere else. Makes me wonder if he does it to a lot of people, and you'd be the first to speak up!
