Having quit a horrible pizza job i will suggest this method. Look at the schedule and find a night that is going to be crazy busy, or a night that is short staffed. Come in on that night with your uniform in hand, set in on counter and say "I'm out bitches" and leave.
As much as I wish I could do that at the pizza shop where I work, it would only punish my coworkers. They're some of the best people I know. It's the greedy psycho owner/boss that deserves it.
As someone who suffers anxiety, I know where you are coming from. Write out a resignation letter at home (be nice and give 2 weeks notice, ie. I will be resigning from my position as of August 19th, 2015). If you have to, simply hand the letter to your boss then head back to work. Do not let them talk you into staying if you really don't want to, but do not be rude. Always leave on good terms! Good luck.
That attitude wont get you around. Time to gain some confidence & tell the manager straight up that you no longer want to work there. Besides what's the worst that could happen? he will be like 'okay, no worries here is ur last paycheck, bye' give it a shot. Know what ur worth & never settle for less than what you deserve.
