I have long thought that elevator Open and Close buttons should always include the words, not just the icons. The icons are useful in that you needn't know the local langauge, but in most instances having the words means this sort of thing would happen less frequently.
She was going to commit bank fraud, so you were a hero by frustrating this arch-criminal!

It's not that you don't notice the icons, it's just that in a split second it's much easier to distinguish two four letter words than some symbols.

Give the guy a break. It was an accident. He was panicked trying to keep the door open for her and pushed the wrong button. It's hard to think clearly when you're rushed.
