Seriously, this needs proper documentation incase they escalate. Maybe ask your manager what the rule book says about documenting, monitoring and safe guarding both you and the client just in case anything legal happens because of it. Phrase it as you wanting to make sure *she* is covered and in the green if anything happens.
To piggyback off the previous points, in addition to escalating to management, if they’re not helpful, go to whatever state mental health boards oversee this kind of operation. Some states don’t have much of a functioning government, but if I understand correctly, Minnesota is usually pretty on the ball.
My addition is, If the supervisor blows something like this off, then what else will they ignore or sweep under the rug. If you can't resolve this cleanly, start looking for another job.
You shouldn't have to put up with harassment from anybody at your work. Report the resident and supervisor to HR. Document everything that has happened and will happen.
