Birth control does not always work. My first child was the depo baby. The one im expecting now is the patch baby. And yes we used condoms.
First of all it happens all the time. Second I have been on birth control since I was 17 due to horribly painful periods. Birth control DOES FAIL. I have never in my life had unprotected sex until I was ALREADY PREGNANT with my second child. So stfu.

If used properly, the Depo-Provera shot is more than 99% accurate and condoms are, for men, if used properly, 98% effective. For women, if used properly, are 95% effective. Third of all, learn to use the reply button and learn to properly use all forms of birth control. BTW, obviously there is a small chance that you can get pregnant while being on birth control, but I'm more than 95% sure that you're full of shit.
Actually it's more common than you think. I've known lots of people who used 2 different birth control methods including myself and still ended up pregnant and yes we used them correctly.
