Explain your situation to your professor and see if they're willing to work with you. Then go to whoever handles disability accommodations and see if there's anything that can be done to help you with this issue (in general, not just this specific class). And finally, go seek out a psychiatrist and work on your anxiety, because this sounds like absolutely no way to live.
This sounds like it may be executive dysfunction, a common symptom of ADHD, ASD, OCD &/or depression, none of which anyone chooses to have. I second the suggestion to seek disability accommodations & psychiatrist. It may be possible to improve your productivity & mental health with medication as well.
You need to trade in your procrastination anxiety for the type that causes you to obsessively over-study.
Talk to the professor, nearly everything is negotiable. Also they might just decide to curve the test. I've had professors curve by +20% for the whole class if the highest grade was only 80%. Worst case maybe drop and retake if you can still avoid the fail on your record.
