Why do people think its the OP's fault? Stop assuming its the women and maybe the person in the van caused it.
Because when you rear end someone, that means you weren't paying attention.
collisions are both drivers' fault. even if the other driver caused the collision, you should have tried to prevent the accident, thats why its called DEFENSIVE driving.

Your mum was right, too. Being a capable driver is a completely different thing than having infinite supplies of concentration. Drive for an extended period of time, and you /will/ lose your focus. This is why you should take a break every now and then.
