People who keep all their passwords in one notebook, usually entitled passwords, are like candy to the various thieves of the world. Not just any candy, though. Christmas candy. From their golden year. Even if they haven't reached their golden year, the metaphorical candy dropped out of the sky from the future, just so that they could compare it to the moment they came across someone who had a notebook with all of their passwords. Oh, also, OP...phone company fail. :/

Ever heard of going to the store...to get a new phone...and the password thing, thats how cell phone companies always have done it...go to the store and get a new one. you'll get it faster anyway...oh and how about make a password you remember??

If you have a SIM card in your phone, try popping it into a friend's cellphone who is on the same network (in case there are any hardware restrictions built into the phone). Then go back and have the password sent to your phone, and your life should be unfucked.
#3 and #4, you aren't very bright. I can see it now "Today, somebody stole a notebook where I had written all my passwords. Now I cannot access my e-mail, my bank account has been hacked, someone charged $5000 to my credit cards via PayPal and my phone has been compromised. FML" And to the original poster, either swap your SIM card into another phone or go visit/call your provider, this situation is fairly simple.
