Time for a new toothbrush
I agree with #1... New toothbrush and maybe keep it on your dresser or in a drawer? Maybe write your name on it with sharpie... Rewrite it if the ink comes off eventually. Regardless you should switch your tooth brush every couple of weeks. Lots of bacteria on tooth brushes.

Well it seems that you would be using a different one as well...so just stick with that one
leave the reappeared one and use the one you have now... and BTW..teach a good lesson to your roommate
Payback time. Go away a few days take their underwear put back into laundry pile when you return. You won't use of course but if they ask agree you took then and if they say that's disgusting then explain taking your tooth brush is disgusting as well. But do tell truth you hadn't used then just took then to teach a lesson on hygiene and that not all sharing is good.
I'm sorry, but how is this fml? This is really petty. Watch the news, people have real problems. It's not even amusing. Buy a new one, they're real cheap. Perhaps he thought it was his? So not that big a deal.

Ah, but the real concern here is this: if the roommate took the toothbrush camping, then maybe he was under the impression it was actually his own. In which case, the roommate could have been using OP's toothbrush for a long time...
That's one of my fears, people using my toothbrush, I just can't stand that thought. Get a new one and no harm done.
