Well I guess there is double confusion here. You didn't realize she had a disease, and she didn't realize you probably never seen someone with Parkinson so you were trying to be nice. I guess a good lesson to learn here is what you can say for future cases for things like this. My guess is that you're young, probably got a job as a cashier or something. This is what I think is good for people to learn and grow. My suggestion is that next time you should try to be more careful with how you ask questions. Like for this woman, instead of asking her what she was disagreeing with, ask kindly, "Is there something I can help you with?" or something along the lines of, "Did you find everything okay?" That way it can't be taken as offense and at the same time you can see what she is disagreeing on, and she would have then maybe revealed to you that she had parkinson.

I seriously doubt he just said "what are you disagreeing with" in a decent tone. Also you could tell something was wrong with her I'm sure it wasn't a steady slow motion she was making. most likely he jokingly said something like "why do you keep saying no" probably halfway giggling because of the irregularity of her movements. Not an innocent victim. Just a douche with an FML account.
Actually, it wouldn't surprise me at all if a woman with a disease that has obvious symptoms would be over-the-top sensitive about their illness. I'm not saying that's wrong, I'm just saying I think it's perfectly possible - in fact probable - that her upset was caused by her own self-consciousness and not the OP's tone. The OP is employed in customer service and therefore very unlikely to just suddenly get cranky at a customer for not wanting whatever he's got. IChances are he wanted to know what she was disagreeing with so he could offer her something more to her liking.
I like readig arguments then going off on everone who is in them :)

Ouch, sucks fo' you. My great-grandma had that terrible, but it never caused her to shake her head that way. Oh well, you didn't know! I understand her being a bit hurt for the misunderstanding, but no need in talking to the manager about that.
