Wow that used to happen to me all the time at my martial arts class. We had to say yes sir to everyone, and when we had the occasional female sensei, we would accidentally say yes sir.. I mean ma'am.

Or maybe she could take it as a compliment. In the military, you say 'Yes, Sir'. Maybe she thought you had a polite upbringing.

Similar for me, I have my football coach as a teacher, I accidentally answer him with "yes coach" all the time in class

That's actually correct. Sir in that sense is not gender specific. Like chairman is, the (person{male or female} in the) [chair] with the [man]uscript (agenda) .
