Am I the only one who got the impression that the "old Tiffany's box" was from a previous Tiffany's gift he gave her and he has just re-used the same box? If that was the case- I bet Op was ungrateful at his first gift so he just figured if she was ungrateful again it only cost him $15... Well at least something along those lines... OR she demanded something from Tiffany's and he couldn't afford it or he thought they were a rip off and underestimated a woman's ability to spot a fake.

Seriously, OP is upset that she got earrings in a box from a place called "Tiffany's." (yes i know what the store is, but i don't really care about their products) When my fiance and i got my engagement ring, he bought a used ring that i love, but it didn't come with a box from Kay's. If she really loved the idea of the box more than the fact that he thought about her and bought her something he thought she would like and put it in a nice box then let her adore the box. Just like a toddler who gets a nice gift they don't know what to do with but enjoy playing in the box more than with the toy.
