If you had contacted the police when she threatened your life she would have a record, and wouldn't have been able to get a concealed carry permit. Threatening to kill someone is a serious offense.

You need to report that. Making that kind of threat is not funny at all. If you report her directly to the agency that issues concealed carry in your area, they will investigate and revoke her permit, and prevent her from ever getting one again. It's not a joke when it comes to guns.

Did you ever report the threats to the police? Had you, she shouldn't have passed the background check done to obtain a CCW permit.

Also, I recommend arming yourself and taking a course on self defense or your basic pistols 101 at a local gun range/store/whoever offers it. If she EVER shows up, call the police IMMEDIATELY, calmly explain the situation and add the magic words "also, I am armed with a firearm", not in a threatening tone, just calmly inform then of it...the police will be there in a fraction of the time for any other call. Little trick I learned when I took my CCW course.

Get an order of protection against her. I have one against my ex, it makes it a federal offense for him to own or have a weapon in his possession punishable by up to 10 years. It'll invalidate her concealed weapons permit.
