Don't worry about it, my parents are divorced and after spending a week at my dad's, I'll occasionally say "dad" to my mom. It's just what you get used to saying, when she's older I'm sure she'll do it less
My son is 10 and he has called me one of his teacher's names every year and sometimes calls his teacher "mom." Don't get so bent, it's an innocent mistake and really not baby mama's fault either.

Everyone needs to stop assuming that he does not spend enough time with his daughter. I have a 4 year old step-daughter who has called me by her mother's first name on several occasions. My husband and I have primary physical custody of her, your arguments are invalid. His daughter is 3 it's an honest mistake.
When I was 5 my mum got together with my stepdad. It was confusing at first and I accidentally called him dad a few times and my dad by my stepdads name. Some people (like me) really suck with names and at the age of 4 it's really not easy! Don't sweat it, she's probably just getting used to the whole thing :)
Give her a break. She is little, our kids interchange their words like Mom and Dad or Nan and Grandma. Just correct her gently and simply move on in your conversation with her. She doesn't have any idea the emotion it provokes in you.
