If you have a bowel condition such as IBS , and take any kind of prescribed medication for it, then in no way shape or form can your boss refuse you to go home and clean up. They certainly cannot force you to continue working in soiled clothing like that. It's not only unhygenic, it's uncomfortable for you in every way imaginable, and also anyone around you due to the smell. Seriously OP, as disdainful as I am of compensation culture, you have a fairly good chance of getting your boss done for this. It was a horrible and very very demeaning thing to force you to work in quite literally shitty clothing. Even if they had no sympathy for your condition , on the practical basis of not causing mental and physical distress to you and others around you, they should have allowed you to at the very least go home and clean yourself up ,and change clothes before retrning to work to continue your day. If you have to face the public in your job, then forcing you to work in soiled smelly clothing like that can only be construed as a form of cruel punishment, for something you clearly had no control over. That is bullying plain an simple. And they could very likely be prosecuted for it or made to pay compensation in someway. You really should seek some sort of tribunal for this. They cannot legally treat you that way. And as bad as this job climate is in the UK at the moment, If you decide not to go through with any action, and they treat you in a similar fashion again, LEAVE. In fact start saving as much money as possible asap if you haven't already, in the event you may be forced to such a situation by their lack of tact and understanding. I know I wouldn't be able to put up with that for very long.
Always keep a spare clean outfit in your car. You never know when you may need it!
As said by other people: if this is a "familiar" pain and you had to run to the toilet... If you have a condition they can't refuse you. If boss doesn't know about condition or why you're asking to go home, they should try to be compassionate anyway if you make it clear it's urgent...
