Relationships aren't about "one last chance" they suck and take work, none of this ultimatum crap, nothing is ever permanent, stop acting like a child and just talk to him. It's not his job to fix it it's BOTH of yours.

Dude chill, he obviously needed space. Give him a couple days alone. If by the end of say, 3 days you two talk and the feelings are still there THEN you can try to fix it. If not, then move on. Someone better will come along.
