He was asking to see if either he could be of assistance, or if there was anything seriously wrong. Women complain about there not being any nice guys then kick the nuts of the few they find. Nice guys are probably afraid to approach women cause they don't want to be punched in the face or kicked in the nuts.
There is nothing wrong with askin' the obvious question. Hell she could have said yes she is ok and then kept cryin'. I could've easily been the op, comin' up to a stranger askin' are you ok, do you need anything. Is it annoying when people ask questions like that, yes. But I'm not five years old and I don't get to throw a temper tantrum 'cause I'm upset. If I was cryin' and someone asked how I was they might catch some sarcasm, but I'd thank them and either accept/decline their help.
