This sounds more like a morbidly obese problem, not an out of shape problem. I'd start with dieting with the progress of moderate exercise
Or a problem with the lungs. Or a general lack of energy. Or chronic pain.

Sorry if this is long but trying to be actually helpful. I read something recently about self-improvement where it's best to set your goals LOW, as in achievable. The example I read was do two pushups after every pee. Stick to it, and feel good about it. And build from there.
