It probably was tested, it sounds like op just made a typo, or skipped a line. not hard to do.
That's why you put all the changes you want to make into a shell script, and run that. In other words: don't EVER make the changes manually on a production system.
I have found myself in similar situations like this, and its feels very guilty when someone takes a stand for you. Your supervisor is a gem of a person and you should apologise to him with all your heart. So you made a mistake, its natural!
Why should op apologise to the supervisor? Op didn't punch him... Op is not responsible for the admin guys total overreaction.
