Her real name could be hard to write/pronounce, so she goes by an easier nickname for everything, not really uncommon.

I honestly feel bad for your girlfriend. you don't seem like the type to remember someone's birthday. do you know the anniversary? that's her name, and contrary to romeo and Juliets infamous quote, there's a lot in a name.

depends on how long it's been there are some close friends that can't remember my name meeting (12 years of friendship and only one of those ever having an actual name involved) I've forgotten girls names before...not hard when she says her name a total of once (when you meet) and then is always called something else with no association with her name
