I once forgot my dog's real name because we always call her by a nickname. It may not be common, but it does happen.

We gave my dog a nickname and because of it we forgot how to spell her real name. (She had a uniquely spelled name)

Has nobody considered that a nickname might just be a short version of a name that is possibly hard to remember and spell? Maybe she specifically told him to just refer to her with the short name, not knowing that he would entirely forget her full name...
