Lol you should really tell him
You should just tell him.
This is why it's a terrible idea to get Japanese words as tattoos also, unless you're fluent enough to understand or someone you trust can translate it.
Poor bloke! Good luck to him
Yeah, when getting a foreign glyph Tattoo, painting, coffee mug, whatever- done, it's wise to have someone who can read the language and is on your side as a backup. A FOAF went to a tattoo parlor and wanted the Glyphs for Wisdom and Courage in Korean Tatted on his arms. He took a schoolmate who read Korean with him, and lo and behold the Tat artist was preparing to put Stupid and Ugly on him. Said Tattoo artist got his ass beat thoroughly. The friend offered to write the glyphs beforehand but the dude was curious as to whether the artist was honest or not.
That last part would have been a smart move in any case. And good on your FOAF for beating that jerk. Do tattoo artists show what they would do beforehand?
then don't
That sucks. Don't tell him. He'll just take it home and enjoy it and possibly show it to friends and family. I've been to japan and most of the people are very nice.
You have to tell him. Otherwise you're the friend who reads Japanese who didn't give him the heads up. Better he feel like an idiot for a minute than look at you differently for the rest of your friendship.

that's really messed up but I don't think I'd have it in me to break it to him either.
