The ****** must have sucked up the tampon and sucked up some panties with it. That is the only reasonable explanation I can come up with in order for the wedgie to have been bad enough to pull a tampon with it.

The string might have ridden up the back. Which means that it was long enough, therefore the tampon wasn't in as much as it should have been.

It happens (even when you have the tampon inserted properly). The string may not always stay right where you want it to so, if you "pick" a wedgie, you might grab a small piece of it. However, I've never known anyone to tug hard enough to rip it out completely!
