Your tampon must not have been in all the way. When used properly, it's pretty damn difficult to pull one out accidentally.

As others have stated, she either had a frontal wedgie (camel toe) or the string somehow meandered to the back. Either way, accidentally pulling a tampon out is possible - but only with some vicious wedgie-picking.
