Because when it comes to specialists, it is usually very difficult to get an appointment (they tend to be booked full for several months ahead) and it is standard procedure for appointments to be booked 3-8 months before they take place. There's a lot of very frustrating waiting involved, even when the need to get in is rather urgent.
YDI for not just going. if it was really important and hard to come by, just go, face the consequences later. If your boss can't understand, maybe you shouldn't be working there in the first place?

As soon as the schedule comes out that is it. It is up to employees to get their shifts covered. Ie: it is illegal for the boss to change the schedule once printed. (At my job). My back specialist is booked months in advance but when it was urgent (car accident) they came in early (6am) for me.
