you need to get the PUK code from your service provider, just call in and ask for that info. I work in the cell phone retail market in Canada and deal with this all the time.
I got my PUK code when I bought my SIM card.

HAHAHA. I did that once--the phone worked fine, but the SIM card was locked and I couldn't make or receive calls. Took three hours with the *enterprise* support to fix. Enjoy.

I've done that before, it's not something that was in the manual for me. If you call your service provider though they usually give you a code to unlock it again. Just don't try guessing that too otherwise your phone will permanantly lock up and you have to mail it in.
