And why would you not point out that fact that it has been properly documented above on the page? If you did your job well and proper you shouldn't let people treat you as if you didn't. No need to be smug or dramatic about it. I would think a big red arrow from her rant to your log should cover it. Then consider the problem corrected..
How do you dare to be rational and use some logic?
I like to think of it as supervisor reeducation. Most my supervisors don't come trained to my liking, a little tweaking here and there and they're much more tolerable.

Communal notebook?? Can you not just send out a department-wide email or email IT? What kind of place are you working at?
We have communal notebooks/whiteboards at both my jobs. I work in service so we don't all have our own computer or email to access daily.
