I hate people who think their lives -- and perhaps life in general -- is some sort of exclusive club with very specific rules. Rule 1. You're not allowed to say, "You look like your mother died or something!" Why? Because his mother may have actually died. My response: So what? It was a joke, and he says, "Yes, she did die," and you promptly change your tone, "Oh, I apologize. I'm sorry for your loss." The end. People die all the time. You can't be expected to always be on your toes, worried that you might offend someone.
