To make a joke relating to death when someone is purchasing a gift for someone elderly is not at all uncommon. Doing so does not make the OP wrong in any sense of the word. She has no way of knowing what type of sense of humor the customer will have. If they are upset with your suggestion, simply apologize and make a more serious suggestion. No need to get bent out of shape over a harmless joke.

You should have shown her a selection from the "your'e hubby's got cancer and is gonna die" catalog. This would have certainly helped her make her selection, and you probably would have sold a big fancy cake.
