Ok, so nobody may support how I see this, but at least hear me out. Though the boyfriend may have gone out to get a bite to eat and not said he was going to be gone for hours, it doesn't mean he's a bad guy. Its possible he went to go get the food, which was also at a bar, and as hes snacking away the anxiety of his girl being in a sickbed, some of his friends show up. Now he's having a good time clearing his head over the health of his woman, and he has forgotten to notify her probably assuming she's asleep or something since that is a common thing for people to do after surgery. After his mind is cleared he checks the time to find he's been out 3 hours longer than he meant and heads back. Now, I am sure as heck not taking the boyfriends side, I am saying he should have been more responsible. But it may not entirely be in conscious thought.

I would think the emergency surgery may have put him under a lot of stress. He's was probably worried and needed to let off some steam. Feel better OP

If he is out drinking, it is presumably night-time which would be past visiting hours. Therefore, he isn't a mondo-douche bag for going out. On the other hand he might be cheating but we won't ever know.
