It's possible. I earned a law degree in English from distance learning in Asia, never lived a day of my life in the US. When I migrated here, although I spoke better English than most of Chinese immigrants, my lack of common knowledge surprised so many people when I took a job in an Asian restaurant. As Chinese, I didn't know what LowMain was, nor Pasta.
she probably couldn't understand what you were saying. probably due to an accent. I know that has happened to me before. I find it hard to believe that someone old enough to get a job has no idea what lettuce is.
I am losing hope as well. Why would anyone choose to eat burgers from a fast food joint, and even worse, without lettuce. The cashier was probably just struck by surprise.

I'm not sure what fast food "joint" (nice Americanism) in the UK actually sells burgers with lettuce? Maybe that's why she was confused, because the burger doesn't come with lettuce? Or she was surprised that an adult can still be fussy about lettuce? Or you mumbled it? I'm sure it's quite loud behind the counter in one of those places.
Gee give the girl a break, she might have been foreign, or it was noisy or she was tired or he mumbled, there are plenty of reasons why she would have misheard him.
And yet, you're the one asking for the only minuscule bit of healthy food to be removed from the burger, so really not much better.
It's not about the calories, it's about the fact that the lettuce is the only thing in that burger that doesn't increase your cholesterol and risk of heart disease. It's also got fibers, vitamins and minerals (unlike the rest of the burger, which is mostly fat, carbs and protein). Of course, a tiny leaf of lettuce is not going to make a burger healthy, but still it's a teeny tiny bit better to eat it with it then without it.
Heh, I've had to explain to some very confused young waiters that I am allergic to wheat. And that bread has wheat. Therefore I cannot eat bread. Or breaded chicken wings, breaded seafood, onion rings, turkey stuffing, flour thickened gravies, etc. Not their fault I guess, I have a cousin who grew up on a farm and doesn't know the meaning of the word 'fallow'.
