maybe the fact that he was smiling made it worse...i would rather have an angry customer do that... Its less rude and more expected

Something similar to this happened to me at work last year. This man ordered a ham sandwich and his friend ordered a chicken sandwich. When I handed them over to both men I mixed up the names and called the ham one the chicken sandwich and vice versa. The man with the ham sandwich ended up getting mixed over his sandwich and said he ordered chicken and that I didn't listen, but at first I thought he was joking because he was laughing. Then he stopped laughing and started yelling. My coworker even said he'd ordered ham. I could've changed the order since they're the same price but nope.
