You were insensitive...next time ask first...even if he didn't say it, I'm sure it hurt him.
I don't think anyone would equate blowing up cheeks to a disorder, she probably assumed he was flirting with her.

I have several relatives who've had strokes and I think I still would've done what you did OP... the puffed-cheek thing has to be a pretty rare side-effect of strokes, you had no way of knowing (though FHL as well, a stroke at 12?!)
Although uncommon, it's not nearly as rare as you'd think. Roughly 300 kids (under 18) have a stroke a year. That's almost 1 a day. I've met several kids who have had strokes even as young as 7. In fact, there was one boy I met who had a stroke in the womb. So it's less common than you'd think/like Sources: I had several strokes over the ages of 13/14
I think you handled it well considering you had no idea. As long as you weren't rude about it when you asked then I'm sure he didn't take it offensively. Things happen.

I wouldn't worry about it, I mean yeah it's embarrassing for you, but it doesn't sound like he was upset or anything. If anything, it's probably a common occurrence for him.
