Idiot. You don't punish a puppy for going to the bathroom in the wrong place. You teach them that it's bad to GO TO THE BATHROOM. Clean it up if they make a mistake and PRAISE THEM when they do their business outside where they're supposed to. You should take a puppy out every two hours or so and every time they eat or wake up from a nap and especially if they come to you acting like they have to go. Here's a tip: Install a bell on the door the puppy can reach. Each time you take the puppy out, ring the bell. The puppy will associate the bell with going out and going to the bathroom and they'll start to ring it on their own when they have to go. (Be prepared for a puppy to ring the bell with false alarms at first, but keep at it. By the time the pup is grown up he or she will associate the bell with relieving himself/herself.)

Yeah, I agree. I snap my fingers once really loudly and just go "NO!" in a stern voice. Eventually all I have to do is snap.
You deserved it!! You shouldn't even leave your new puppy alone where there's carpet. It's just a little puppy- she couldn't help it. It's called training pads. Obviously your trainer's an idiot. And shouldn't be a dog trainer, if their going to give bad tips, that will scare the dog that much. Do the puppy a favor, get a new trainer...
That actually is a legitimate method for training dogs... Except yours, obviously.

You should find out when your trainer ISNT tripping on shrooms, then get tips from him/her.

Best response is to immediately take the puppy out to the place you wish it to use as a washroom, usually they will actually go a bit more (or maybe even poop like in your case) and if they do go to the bathroom reward them with a treat or praise and a cuddle. They catch on quick and start letting you know that they need to go out. Cans of pennies are best reserved for moments like inappropriate chewing and such. Good luck on the rest of your training!
